April: Audit Resistant Care Plans – Critical Coding Concepts

*Description: 

ICD-10 coding of patient conditions, diseases, and illnesses is a critical element for establishing the foundation for care in a Medicare Part A home health episode of care.  Although the laws provides for the physician to provide the diagnosis codes, home health providers do not always find this to be true.  OASIS assessment strategies allow diagnosis information to be obtained through assessment with verification from the physician.  To accurately complete this component of the OASIS data set, the clinician does not have to master ICD-10 coding, however, does need to provide ample supportive documentation for determination of a condition, disease or illness and any related conditions to ensure code selection accuracy.  The admitting clinician is responsible for establishing the primary reason for the episode of care, as well as any other appropriate diagnoses for inclusion.   

This webinar will provide attendees with clear guidance for clinicians regarding the role of their documentation in supporting the ICD-10 coding of a Medicare Part A home health episode of care.  Attendees will have the opportunity to review and assess what meets “sufficient” documentation standards for ICD-10 coding, and reviewers of clinical documentation will be provided resources to facilitate efficient communication with field personnel when documentation falls short.

Development of an interdisciplinary care plan that reflects key elements outlined in the current Conditions of Participation (CoPs) requires a patient-centeredness not currently optimized in many electronic medical record (EMR) systems.  Frustration over “cookie-cutter” care plans is not a new phenomenon in the home health industry.  To complete this component of care planning and coordination requires a fusion of all discipline-specific findings reflecting a “one beneficiary-one care plan” mindset. Key to any home health plan of care (POC) is the incorporation of measurable and meaningful goals expected as a result of skilled service(s) evidence-based/best practice interventions.  These goals and interventions establish the framework for determination of discipline utilizations.

Our July Webinar covered the “nuts and bolts” of the 2019 Proposed Rule and now we need to dig deeper into the implications for clinical decision-making. The home health industry has a history of following perceived payment “incentives” as evidenced by shift in utilization and/or coding each time modifications have been made to the prospective payment system. This cannot happen again with PDGM or it will confirm the suspicions both CMS and MedPAC have had for years – that agencies care more about payment than patients. This session will focus on strategies for assessing current risk areas and preparing for 2020.

This session will put payment items into a larger context to ensure overall consistency when collecting and reviewing data.
