  • Process quality measures evaluate the rate of home health agency use of specific evidence-based processes of care. The standardized home health quality process measures focus on high-risk, high-volume, problem-prone areas for home health care. These include measures pertaining to all or most home care patients, such as timeliness of home care admission, immunizations, and use of risk assessment tools (e.g. for falls, depression). As well, there are measures for specific diagnoses (e.g. diabetes).

PROPer OASIS C2 Process Measures - 2017

Product Description

2017 PROPer OASIS C2 Process Measures

Ensure your front line staff consistently and correctly capture OASIS data!

This educational series uniquely provides clinicians with OASIS into 4 key areas, making the “big picture” of this data set relevant and understandable:

Process measures

Risk adjustment

Outcome measures

When leadership loses sleep over latest agency publicly reported outcomes and Star Ratings numbers, OASIS training continues. . . . and there are many available options when it comes to OASIS education . However, do training options really speak to the front line clinician?

OASIS C2 data is the PROP that drives an agency forward.  If one of the blades of a prop is not working properly, the boat will still move forward, but will lose power and efficiency. Series and individual sessions are provided as recordings and are priced per agency.  Keep and use as often as you need to train your current and new staff.

Session Description:

Often identifiable by “yes/no” response options, OASIS C2 items that create process measure data can cause anxiety in two key areas – is there documentation to support the selected response and the negative perceptions if “no” is selected. This session will assist data collectors and reviewers with making peace with both issues and ensure accuracy.

Session Objectives:

ž  Differentiate between a process measure and an outcomes measure.

ž  Define best practice and their relationship to OASIS process measures.

ž  Connect care planning to issues identified by process measures.
